Forms can be accessed on the Graduate Studies website (Graduate Program Forms).  After admission to the program, each student is required to complete a Graduate Form I, which lists the courses the student plans to take, thesis topic, major advisor, previous academic work, transfer credits, etc. It is submitted to Missouri S&T to be approved by the major advisor, department chairman and vice provost of graduate studies.

When approved it becomes the student's formal course of study which must be followed in order to receive the master's degree.
NOTE: It is important for the student to have the Graduate Form I completed and approved early in the program to ensure that all courses taken will be allowed as credit toward a degree. Changes to the program must be requested on the Graduate Form I-A.
